The Hotel Stay Agreement outlines the terms and conditions for your dog’s stay at the Boise Dog Hotel. Please read this form carefully, which requires you to sign to all conditions for you dog’s stay at Boise Dog Hotel. You must include the name of a person who can act on your behalf in the event that we are unable to contact you during an emergency. This designated person, by you, may be asked to provide ID if he or she picks up your dog. If anyone other than you or your designated person comes to pick up your dog, we will not be able to release your dog without first speaking with you. Please return this form to us in advance; otherwise, you may bring it with you when you check-in.

We will perform a Snout-To-Tail Worksheet on your dog while at Boise Dog Hotel. We perform this for all hotel guests. This worksheet is a compilation of all of the weekly checks that all owners should do, but that can often get missed or overlooked for a multitude of reasons. It is a thorough physical examination of your dog followed by a recording of all of the vitals for your dog. The Dog Profile form gives us an opportunity to better understand the needs of your dog while at Boise Dog Hotel. You can describe the details of your dog’s history, health, and behaviors and in turn we can customize our care and attention to your dog while staying at the Boise Dog Hotel. Boise Dog Hotel requires that the Dog profile form be completed at your dog’s first check-in and updated annually.
